In order to allow us to provide the best available support for you we need to know about the nature of your disability and how it might affect your training.

This will not affect judgements concerning academic suitability for the course, and will be treated confidentially. It will enable data to be collected from which the progress of equal opportunities for disable students in higher education can be monitored. 

As a professional body, the PGA takes protection of personal information seriously. We will hold information relating to you and strict accordance with the Data Protection Act 1998. The information we hold on you will be shared with the University of Birmingham and will be used solely for the purpose of your training and will not be sold or passed on to third parties, either by the PGA or the University of Birmingham.

You are required to complete a disclosure application form for the PGA to undertake an enhanced check (Disclosure and Barring Service (DBS), Protecting Vulnerable Group Scheme or, Garda Vetting as applicable) on your background prior to undertaking the Training Programme. A Criminal Records Bureau Disclosure Application Form and Guidance Notes will be sent to you when your application has been accepted. You are required to complete, sign and return this form together with the necessary enclosures.

I declare that the information I have given is true and complete and, being admitted to be a student at the University of Birmingham, promise to obey the Charter, Statutes, Ordinances, Regulations and Codes of Practice to submit to discipline and uphold its honour and credit to the best of my ability, both while a student and if admitted to a degree as a graduate. I understand that any mis-statement by me about my qualifications or circumstances may lead to the termination of my registration as a student. I agree to the University of Birmingham processing personal data obtained as part of online registration, or other data that the University may obtain for me or other sources in accordance with the Data Protection Act 1998. I agree to the processing of such data for any purposes connected with my studies, my health and safety, or any other legitimate reason and to the publication of my degree classification and my studies. I understand that my academic data may be disclosed to organisations acting as my sponsor.
